CHAPTER ONE

INTRODUCTION

Background of the Study

Telemedicine is defined as the delivery of health care and sharing of medical knowledge over a distance using telecommunication means (Eriotis et al., 2018). Telemedicine has grown exponentially following the advancement of technology, internet penetration and a need for a more effective healthcare delivery system. Countries like the United States, Canada, and various European nations have integrated telemedicine into their healthcare frameworks, offering services such as virtual consultations, remote monitoring, and tele-radiology (Dorsey & Topol, 2020). Telemedicine has been in existence for decades but it became more prevalent during COVID-19 pandemic in 2022-2021, which necessitated the minimization of in-person interactions to curb the spread of the virus. The adoption of telemedicine in developed countries has proved that telemedicine can reduce healthcare costs, enhance patient satisfaction, and improve health outcomes. Furthermore, in developing countries, telemedicine has been seen a a bridge between the urban and rural areas to access to healthcare delivery. Successful telemedicine initiatives in countries like India and Brazil have shown improvements in healthcare delivery, particularly in remote and underserved communities (WHO, 2019).

In Nigeria, with a population exceeding 200 million people, the country faces significant healthcare challenges, including inadequate infrastructure, a shortage of healthcare professionals, and unequal distribution of healthcare services across urban and rural areas (Adepoju, 2020). The country's healthcare system is characterized by insufficient infrastructure, a shortage of healthcare professionals, and inequitable distribution of medical resources. According to the World Health Organization (WHO), Nigeria has one of the lowest doctor-to-patient ratios globally, with less than four doctors per 10,000 people (WHO, 2020). The healthcare challenges in Nigeria is even worse in the rural areas where the access to healthcare is often limited due to lack of medical resources and medical personnels. Thus, the emergency of telemedicine to help mitigate these challenges and proffer solutions to the challenges faced in the Nigerian healthcare sector, connect patients to doctor with easy, and facilitate general access to healthcare when needed. 

Despite the exponentially growth in telemedicine globally, in Nigeria, this growth has been slow, often met with distrust, unawareness, lack of technological know how, etc. Different initiatives have been put in place to demonstrate the potentials of telemedicine in Nigeria, the University of Ibadan and the University College Hospital (UCH) Ibadan have collaborated on telemedicine projects to enhance medical education and patient care (Oluwafemi & Samuel, 2019). Other private programs including the telecommunication sector are beginning to explore telemedicine solutions. 

However, the implementation and practice of telemedicine in Nigeria encounter several obstacles, including technological limitations, regulatory issues, and socio-cultural factors (Eke et al., 2021). The country lacks comprehensive policies and guidelines to govern telemedicine practices, leading to concerns about the quality, safety, and legality of telemedicine services (Adebisi et al., 2020). Additionally, there are socio-cultural barriers, including resistance to change among healthcare providers and patients' preferences for traditional in-person consultations (Ekpenyong, 2020).

1.2 Statement of the Study

The adoption and implementation of telemedicine in Nigeria has the capacity to bridge the healthcare gab facing the nation, it proffers solution to Nigeria's healthcare challenges. One of the foremost challenges plagueing the penetration of telemedicine in Nigeria is the lack of necessary technological infrastructure needed for the advancement of telemedicine. Reliable internet connectivity is critical for effective telemedicine services, yet many parts of Nigeria, particularly rural areas, suffer from poor internet access. According to the Nigerian Communications Commission (NCC), only about 50% of Nigerians have access to the internet, with rural areas experiencing significantly lower connectivity rates (NCC, 2021). There is an absence of comprehensive policies and guidelines to govern telemedicine practices, leading to uncertainties about the legality, quality, and safety of telemedicine services (Adebisi et al., 2020). Also, without patients acceptance of telemedicine, it would not succeed, thus, some patients and even healthcare practitioner have resistend the advancement of telemedicine, preferring the traditional in-person consultation rather than remote interaction.

Furthermore, another problem plaguing the advancement of telemedicine in Nigeria, is the lack of training which can result in suboptimal use of telemedicine platforms, affecting the quality of care delivered and undermining confidence in telemedicine as a viable healthcare solution (Adepoju, 2020). The lack of robust data protection laws and cybersecurity measures in Nigeria exacerbates these concerns, making both patients and healthcare providers wary of adopting telemedicine (Oluwafemi & Samuel, 2019).

Despite all these problems facing the advancement of telemedicine in Nigeria, it still has potential prospects in bridging the healthcare service gab in Nigeria, therefore promoting easy access to medical care, therefore, there is a need to examine the problem and prospect of telemedicine practices in Nigeria.

1.3 Objectives of the Study

The primary objective of this study is to examine the problems and prospects of telemedicine practice in Nigeria. The specific objectives are to:

Determine the extent telemedicine is adopted in the Nigerian health sector.

Evaluate the need for the adoption of telemedicine in the Nigerian health sector.

Investigate the problems associated with the adoption of telemedicine in the Nigerian health sector.

Assess the prospects of telemedicine in the Nigerian health sector.

1.4 Research Questions

These research questions will be answered in this study;

What extent has telemedicine been adopted in the Nigerian health sector?

Why is there a need for the adoption of telemedicine in the Nigerian health sector?

What are the problems associated with the adoption of telemedicine in Nigerian health sector?

What are the prospects of telemedicine in the Nigerian health sector?

1.5 Research Hypothesis

Here are some research hypothesis relevant to the study

H0: There no problems associated with the adoption of telemedicine in the Nigerian health sector.

H1: There are significant problems associated with the adoption of telemedicine in the Nigeria health sector.

1.6 Significance of the Study

Significance of the study on the examination of the problems and prospects of telemedicine practice in Nigeria would contribute to existing literatures on telemedicine, it will also aid other researchers in there literature review. 

This study will also shed more light on the problems and prospects of telemedicine practice in Nigeria.

This study would also help develop strategies to mitigate problems of telemedicine practice in Nigeria.

Scope of the Study

The study would focus on the examination of the problems and prospects of telemedicine practice in Nigeria. Empirically, this study will determine the extent telemedicine is adopted in the Nigerian health sector, evaluate the need for the adoption of telemedicine in the Nigerian health sector, investigate the problems associated with the adoption of telemedicine in the Nigerian health sector, and assess the prospects of telemedicine in the Nigerian health sector.

This study would be carried out in Nigeria

1.8 Limitations of the Study.

The study may not be carried out for an extensive period of time to examine the problem and prospect of telemedicine practices in Nigeria. The sample size is also limited to Nigeria which may not reflect the overall examination of the problem and prospect of telemedicine practices in Nigeria.

The study relies of self-reported data, which is subject to biases, errors and inaccuracies.

1.9 Definition of Terms

Telemedicine: The use of telecommunication technology to provide clinical health care from a distance.

Healthcare Provider: An individual or institution that provides medical services to patients.

Infrastructure: The fundamental facilities and systems necessary for the operation of telemedicine services, including internet connectivity, telecommunications hardware, and software.

Adoption: The acceptance and integration of telemedicine practices by healthcare providers and patients.
